Job Description

  • Conduct thorough patient assessments to determine their healthcare needs and develop personalized care plans.
  • Provide high-quality nursing care to patients in their homes, following established protocols and standards of practice.
  • Administer medications, treatments, and procedures as prescribed by physicians.
  • Monitor patients' vital signs and condition, and accurately document observations.
  • Maintain comprehensive and up-to-date patient records, ensuring confidentiality.
  • Communicate effectively with patients, their families, and other healthcare team members to ensure coordinated and holistic care.
  • Report any changes in patient condition to the supervising healthcare provider.
  • Educate patients and their families about their medical conditions, treatments, and preventive measures.
  • Provide emotional support and counselling to patients and their families.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to coordinate comprehensive patient care.
  • Adhere to established clinical protocols, policies, and procedures to ensure the delivery of safe and effective care.
  • Respond promptly to medical emergencies and provide appropriate interventions until further assistance arrives.
  • Ensure proper use, maintenance, and documentation of medical equipment used during home healthcare visits.
  • Stay updated on the latest developments in nursing practices and attend relevant training programs to enhance professional knowledge and skills.
  • Collaborate with other staff nurses and healthcare professionals to foster a positive and supportive work environment.
  • Ensure compliance with regulatory standards, including DHA guidelines, and participate in quality improvement initiatives.

Job Skills

  • Valid nursing degree or diploma from an accredited institution.
  • DHA license/eligibility is mandatory.
  • Should be available for immediate joining.
