Homecare Nurse

Job Description

• Perform physical examination and diagnostic tests
• Observe and record patient behaviour
• Collect patient health history
• Update patient records confidentially
• Counsel and educate patients and their families on treatment plans
• Administer medication, change wound dressings and care for other treatment options
• Read and interpret patient information, making treatment decisions when appropriate
• Responsible for helping patients maintain their health and hygiene in their places of residence
• Assist in improving patient outcomes
• Monitor patient’s vitals and record them in the file
• Maintain cleanliness and comfort of the patient, including bathing, feeding and dressing
• Administering IVs and medications
• Knowledge of how to take care of chronically ventilated patients
• Change the tracheostomy site and take care of the site
• Transporting the patient to the hospital or clinic for an appointment with or without equipment
• Evaluate patient’s well-being, reporting to families and concerned doctor
• Explain treatment and procedures
• Move patients from wheelchairs, beds and gurneys
